Human Reliability Assement Made Clear

Review of Human Reliability Assessment: Theory and Practice by Anthony J. Spurgin
This is a well-written, concise book that provides a very informative and insightful treatment of Human Reliability Assessment (HRA). I would recommend it to anyone who is responsible for design, operation, and management of complex systems. At the heart of the book is a review of some of the catastrophic accidents that have occurred in various industries, such as the Three Mile Island and Chernobyl nuclear plants, the Bhopal chemical plant, and the Space Shuttle illustrating how plant conditions, training, procedures, or management decisions led the human operators to perform unsafe acts. These same events are then re-examined through an application of an HRA to demonstrate how HRA methods can be applied to prevent or reduce the likelihood of such catastrophic events.

Includes up-to-date models like NARA, ATHEANA and SPAR-H

This book was published in 2009 and includes the newer HRA models that are in wide usage today. I found it more comprehensive [AND less expensive] than HRA books written in the mid 1990's. It includes more recent events such the 2004 Columbia accident, and the 2005 Texas City refinery disaster.
The first few chapters of this book provide an excellent overvew of HRA concepts, models, tools and methods.
The chapters I found most interesting were 8 and 9. Each of these chapters analyzes ten major accidents that have occured within high reliability industries, two for each of the following industries: nuclear power, NASA, airline, petrochemical, rail transport.
Chapter 8 covers these accidents as I have been used to seeing them treated: background, sequence of events, analysis details. The writeups are thorough and include event sequence flow diagrams, decision trees and P&IDs as needed for clarity. The coverage is also succinct, requiring only 2 or 3 pages for each event analysis.
Chapter 9 is the most interesting chapter in this book. It brings forward perspectives on HRA I had not previously been exposed to. In chapter 9 Tony Spurgin re-analyses many of the same accidents from chapter 8, this time he applies HRA techniques. For me, seperating out the analyses in this way clarified the function of HRA, and helped me thoroughly understand it's significance in evaluating the human performance aspects of accidents.
I am very happy with this book, I recommend it to my engineering and management friends.

As a Japanese PRA engineer

I met this book when I worked with the author in a PRA project. It was before the accident in Fukushima. This book moved me, so I recommended it to young engineers to think how we should treat the risk.
The great point of this book is that the issues described in this book seem to be related to the accident in Fukushima even though it was written before that. Of course, the methodologies will be kept updating, some new methodologies
will be developed, and the author will also keep contributing to HRA, but anyway, it is encouraging us to tackle on the risk.
Another point is that past HRA efforts and current situation around HRA are generally shown in this book. It means that the book can be used as a textbook for engineers and students who start learning HRA. Unfortunately, I do not
know any other good books for HRA, but I guess there are not better books than this book.
I do not write here about the contents in this book, because I would not like to reduce readers' pleasure to find something new for them.
